Due to new federal legislation designed to reduce paperwork and administrative burden, this notice is to inform you of a change to how Form 1095-C, “Employer-Provided Health Insurance Offer and Coverage”, is being distributed.
Form 1095-C will no longer be distributed in paper form. Instead, it is available to you either:
- Electronically in iSolved (Pay and Tax>Year End Tax Forms in the app or Employee Self-Service>W2/ACA/1099 Forms in the classic version)
- By request at any time to Human Resources

What is Form 1095-C?
Form 1095-C provides information about the health insurance coverage offered during the year. You are not required to file this statement with your personal taxes.
